OHSAA Pre-Participation Physicals are valid for one calendar year from the date of submission.

In addition, the Pay to Participate Form must be submitted along with the fee prior to the posted due date.

Each participant and their guardian is required to read this document and to view the Sudden Cardiac Arrest (SCA) video.
